1. Getting started with a My Prime (ISA)

Declaration & Authorisation

I declare that

• I apply to subscribe for a stocks and shares ISA for this tax year and each successive year until further notice.
• all subscriptions made, and to be made, belong to me;
• I am 18 years of age or over;
• I am resident in the UK for tax purposes or, if not so resident, either perform duties which, by virtue of Section 28 of Income Tax (Earnings and Pensions) Act 2003 (Crown employees serving overseas), treated as being performed in the United Kingdom, or I am married to, or in a civil partnership with, a person who performs such duties. I will inform Scottish Friendly if I cease to be so resident or to perform such duties or be married to, or in a civil partnership with, a person who performs such duties.
• I have not subscribed/made payments, and will not subscribe/make payments more than the overall subscription/payment limit in total to a Cash ISA, a Stocks and Shares ISA, an Innovative finance  ISA, and a Lifetime ISA in the same tax year.
• I have received the current Key Features and Terms & Conditions, or Product Guide and Terms & Conditions and relevant Key Information Documents.
• I agree to the ISA Terms and Conditions.
• I authorise Scottish Friendly Asset Managers to hold my cash subscription/payment, and any other proceeds in respect of my ISA investments and any other cash and to make on my behalf any claims to relief from tax in respect of ISA investments.
• I declare that this application form has been completed to the best of my knowledge and belief.
• I undertake to advise Scottish Friendly Asset Managers Limited in writing if there are any changes in my circumstances referred to above.

Data Protection Act

I consent to the Society using the information supplied on the Application to administer my policy and acknowledge that it will be held and my policy will be processed on the Society’s computer systems and may be used for underwriting or claims handling purposes.

Money laundering regulations

To comply with money laundering regulations, we may verify your identity (or the identity of the payer, if different) by carrying out an electronic check with an independent agency. This will involve an electronic search of information, including access to databases and credit data. By submitting this form you consent to us undertaking this check of your identity.
